Parents/guardians, please fill out the form on this page if interested. Once that information is collected you will be contacted and then provided with next steps. You will also need to:
-Provide proof of a current Physical. (Sophomores and Seniors who have completed this process the year prior should be unaffected as physicals remain valid for two years.)
-Fill out a 2024 Athletic Registration and Medical Emergency Form.
-Register and pay fees for a valid American Water Polo Membership under Kentridge.
